“Amy” is an amazing young lady who came to HCRS in 2007; she had been placed in residential school due to her uncontrolled behavior issues and inconsistent home life and had a State appointed legal guardian. With the help of HCRS, Amy was placed in more stable home environment with a family that she had already established a relationship with during emergency respite care. With natural supports and the supports from HCRS; Amy has been able to establish a safe and more consistent relationship between herself and her biological family; she has been able to maintain gainful employment; and just last year was legally appointed to be her own guardian. Amy started working part-time at a grocery store and a department store. In 2010, Amy successfully completed a 3 month internship as a front desk clerk. This internship opportunity really helped build Amy’s vocational skills and professional self esteem. Since this internship, Amy has flourished at work and socially.

HCRS Complaints & Grievances

Process for Resolving Complaints with HCRS Programs and/or Services

Anyone receiving or participating in services at HCRS, has the right to complain or file a grievance. If you are dissatisfied with our programs or services, we recommend that you speak with your provider or their supervisor to see if the concern can be immediately resolved. If you are not completely satisfied or do not wish to speak with the provider or supervisor, you may file a grievance.

A grievance is an official process where you submit, either in writing or verbally, the concerns you are having with our programs or services.

To submit in writing, simply complete this Grievance Form and mail to the address noted at the bottom of the form.

 

Grievance Process:

  • Grievances must be filed within 60 days of the situation that resulted in dissatisfaction.
  • HCRS staff members will be happy to assist you with the completion of the Grievance form or verbal submission of a grievance.
  • A written acknowledgement will be sent to the person filing the grievance within 5 business days.
  • The Grievance & Appeal Coordinator will work together with the relevant Division Director to review/investigate all grievances from the initial filing through resolution.
  • All grievances will be addressed within 30 days of receipt. The person filing the grievance will be sent a written notice of the investigation and resolution within 30 days.
