HCRS collaborates with the Brattleboro Memorial Hospital (BMH) to provide a full-time Health Coach for the Brattleboro region. Nancy Schaefer, an HCRS staff member, serves as the Health Coach working with BMH and the Blueprint Community Health Team.
Nancy assists patients in designing an individualized patient-centered health improvement lifestyle plan. The majority of her clients are seeking assistance with weight loss. Patients are seen in the office, in their homes, in the community, and, if it fits their individualized goals, at exercise facilities. Nancy works with individuals, youth, and couples.
In her role as Health Coach, Nancy helps patients make healthier food choices, offers taste tests of nutritious foods, and can provide healthy meal cookbooks. In addition, Nancy works with patients to incorporate physical exercise into their daily routine.
In collaboration with the Brattleboro Food Co-op and Hunger Free Vermont, Nancy offers hands on cooking classes for youth patients and/or children of current patients. This six-week series will teach these youth how to make quick and healthy meals at home. The Food Co-op and Nancy are currently scheduling a cooking class for adults and plan to offer a hands-on cooking class for teenagers.
